Internal amalgam optimizations: preserve or replace them? | Dentistry study Stere

A metallic filling can cause concern even if it does not bother. Its color, however, does not say whether the restoration is intact or should be replaced.

The useful question is what would be earned and what would be lost by intervening on that tooth. Removal involves a new processing and can sacrifice dental tissue.

The decision must therefore be built on the visit and clinical history, distinguishing a need for care from a necessary replacement only for fear or aesthetics.

What are the fillings in amalgam

Dental amalgam is a metal material containing mercury linked to other metals, used in restorations already present in many mouths. Evaluating an old filling is a different problem than using new material.

Four items to be evaluated

Conditions of margin

The professional controls the contact between filling and tooth. A simple photograph does not show the presence of caries.

Structure of the tooth

A crack, a weakened wall or a loss of fabric can change the conservative possibilities.

Medical history

Pregnancy, pathologies and suspicious reactions to materials should be reported. They do not transform any existing restoration into an urgent removal.

Expectations

The desire for a clear material is legitimate, but it must be discussed together with the invasiveness and limits of replacement.

Balance with dental models and text that invites you to evaluate tooth, margins and symptoms before deciding.

When requesting a check

Pain to chewing, a missing piece or a recent change deserve control. Even the absence of symptoms does not certify integrity: the evaluation considers restoration and tooth as a whole.

Metal restoration and mucosa pigmentation

A gray spot of the mucosa, sometimes tied to a tattoo from amalgam, is not the filling itself. He has a distinct diagnostic path. Not even a generic metallic taste shows that all restorations are defective.

How to evaluate the tooth

During the visit we examine margins, residual tissue and symptoms; any diagnostic images must answer a question. Ask what concrete defect would motivate replacement and what alternatives are reasonable.

Storage, repair or replacement

Store and control

FDA indications do not recommend indiscriminate removal of integer fillings without a clinical need.

Repair when possible

A circumscribed defect may require a conservative assessment; it is not always equivalent to remaking all restoration.

Replace when indicated

If the tooth or restoration is compromised, the floor considers extension, insulation and suitable material.

Deepening a suspected reaction

Symptoms or injuries attributed to the material should be diagnosed, without proof of DIY removal or promises of «detoxification».

What to do at home

Keep cleaning habitually and note changes. Do not scratch or polish the metal with domestic products. Bring any reports and clarify whether the request arises from a disturbance or an aesthetic preference.

The conservative assessment at STERE

The Studio STERE di Lecce presents among its services the conservative dentistry. A visit can clarify the conditions of the individual tooth before choosing if to intervene.

Frequently asked questions about old fillings

Is a dark shutter always to change?

No. Colour and apparent oldness are not enough to establish that it is defective.

Does the amalgam remove any risk?

No: removal also involves temporary exposure to vapours and loss of dental tissue.

What if I'm pregnant?

Tell the pro. Do not remove intact restorations only for this reason without a medical reason.

Is a stain on the gum the same?

No. Mucosa pigmentation requires a separate evaluation.

How do you decide the new material?

Based on extension, residual tissue, clinical conditions and maintenance possibilities.
